That is the point though, those exceptional items are rewards for being outstanding players. If you take away rewards for being above average, then you effectively lower the standard for the game in general. I say that it is fine the way it is.
The rule of thumb...
Only thing I could possibly think of, would be to allow pinns to write an app, detailing some item, how they learned of it, how they plan to go about finding out its whereabouts (no, not asking some mage/cleric to do the good old locate object), risks and tribulations involved in reaching and obtaining said object. Submitting it to the IMM staff. If they rule that the entire thing is good/balanced/relavent to the current storylines, then a quest (part journeying, part RP, part combat or some other type of test left to the discresion of the IMMs involved) of a seriously meaningful length of time would be undertaken. I'm not taking an hour...I'm talking your *** cheeks going numb from sitting on them so long. There should be serious risk involved with said quest, like death effects on condition multiplied by 4 or more...or even the more daunting (and one I like more) death during the quest = AUTOMATIC CONDEATH! It would make it require exceptional RP, IG fighting skills, An extremely well trained and operated character, a healthy bit of luck, some rope, and real dedication. Obviously this would be a one time only thing per character. I would also say that the item acquired shouldn't be some super-insane-crazy-godly plate of the whale-item, just an item with average or slightly above average stats and perks as most good rares or uniques. SUPER NASTY INSANO items would instantly unbalance the game, especially since they would most likely be snapped onto a pretty filthy character. The opportunity to RP a crazy quest, serious interaction with one or more IMMs focusing solely on challenging the hell out of you, and the prestige associated with your new completely unique hat or sword should be enough without handing out OHKO swords and -1119 vs. spells breastplates.
Even despite all of that, if the IMMys think you are showing excellent RP and really breath life into your character, they will notice. OR the pbase will, which the IMMys will in turn notice. As people have said before, rewards are given out, rarely but still given out, if you earn them. I'll conclude with this, a wise man once said to me, "All the best things in life can't be held in your hand." Transfer that little saying to your character and you'll realize that a new special shiny isn't nearly as important anymore.

It's already possible to earn 3 owner only items, that I am aware of, through questing - one of them possibly being "the best" armor in the game. And... honestly... what's the point of having owner only items if everyone else got them?
Having one indicates quality character, be it description, RP or even PK. Why can't it stay that way?
