I think volcano is in, but not connected. Never seen the ice fortress.
There is a nice fire ball that people use to use that belong to volcano.
Lots of zones with nice mobs and stuff all around like the orcs, but still not connected.
Its a bit frustating finding a new item with a healer just to find out the zone isnt open yet and having to not use them.
I'm all for emerald at least getting a facelift, but not replaced. I like all the interest in building, but PLEASE! include a reason to go there. I've gone exploriing on long hikes south of Rheydin and found a few spots with a few mobs, no stores and a pit. No equipment or ranking to be found there. It's frustrating to explore and find I've wasted time exploring boring areas.
Also, I think some of the 50 areas need to be toned down so that it can at least be entered. Specifically, the Icy area south of the ford. (dunno the name) I think quest areas are great, but there needs to be a sign or mob with a note to spell out the quest. I would have never figured out there was a quest in Winter if groups didn't go there.
One thing that I would like to see is a few more linking areas to make running, chasing more interesting. Northwest of Rheydin and wouth east of Rheydin seems to be needing linkage.
I agree with the linkage, but I like the random crap areas. Gives a place to hide.
Every area has a purpose. Even if that purpose is for me to go to a place noone else has ever heard of and train for 20+ hours without ever seeing anyone on where. 
Talking about crap areas, I must mention the hospital in sharazarde(sp?). Its like someone wrote five room descriptions and copypasted it everywhere. Also far from being a normal hospital, its looks like a mental institution with lunitics running around, people straped to things... not to mention the evil doctor. So why is it in the elf city??
The hospital was one of the newer areas added I believe. I agree it sucks, and I have no idea why they put that in...

The hospital was designed by the Healer Heroine (built/coded in by an Imm) and is meant as a reward for reaching Heroine status. Other examples of Hero areas are the ship in the Dragon Sea and the music box house in Drkshtyre.
EDIT: And they pretty good hiding areas if you know the rooms and your opponent doesn't.
